Mobile Clinic Coming To D300 Schools This Summer

The Ronald McDonald Mobile Care Clinic will offer care to students will Medicaid.

ALGONQUIN, IL — The Ronald McDonald Care Mobile Clinic will be coming to District 300 schools this summer. The clinic is meant to provide easy access to quality health care for students with Medicaid as their primary insurance to assist in obtaining the state of Illinois required school physicals and immunizations, according to a news release. This program does not accept private insurance or payment.

Those who do not have insurance to see if your student may qualify for services can call Sherrie Schmidt at 847-551-8371. To schedule an appointment, call 847-551-8371.

The clinic will be at Carpentersville Middle School from 9 a.m. to 3 p.m. on July 10, at Perry Elementary School from 9 a.m. to 3 p.m. on July 12 and at Dundee-Crown High School from 9 a.m. to 3 p.m. on Aug. 7.

To provide the best and safest care for students, parents will need to fill out the following forms before visiting the clinic:
  • Patient Information (Demographic) sheet which includes consent for treatment
  • Child history form and/or sports history form
  • Vaccine (shot) records (if not provided to the school)
  • Insurance information (Medicaid plan name and ID number)

Completed forms must be returned at least 1 week prior to scheduled appointment. Forms can be scanned to Health.Forms@d300.org or faxed to 847-551-8433 or dropped off to the D300 Central Office.
Your consent for treatment will allow the clinic to immunize your student according to CDC and IDPH guidelines. Please view the following website for information on the vaccines:
