Transitional Care of Arlington Heights Reinvents Post-Acute Care in the Northwest Suburbs

The new Transitional Care of Arlington Heights, 1200 North Arlington Heights Road, offers comfortable, holistic and clinically proven short-term rehabilitative care that helps patients who are recovering from acute and chronic conditions bridge the distance between hospital and home. Managed by the Illinois-based Transitional Care Management, the $27M, 120-bed center features primarily private suites and is the fist purpose-built, stand-alone, post-acute care center in the Northwest suburbs.

The new post-acute recovery center will offer an alternative to nursing home-based rehabilitation services and provide short-term skilled nursing and physical, occupational and speech rehabilitation for patients who are transitioning out of the hospital or other acute care setting.

“Bridging the gap between illness and recovery requires state-of-the-art technology, efficient and effective clinical expertise, caring support and an overall environment that promotes healthy wellbeing,” said Denise Norman, President of Transitional Care Management. “Our goal is to build the future of post-acute healthcare by providing hotel-like accommodations, specialized care and hospitality services that help people get back home as quickly, safely and as comfortably as possible.”

Transitional Care of Arlington Heights features:

Primarily private suites with private European bathrooms and thoughtful convenience like wider beds, in-room refrigerators and safes, flat screen HDTVs and high-speed, wireless internet access.

Two dedicated therapy gyms featuring over 5,000 square feet of rehabilitative space, state-of-the art rehabilitation equipment and 12-hour-a-day, seven-days-a-week, 365-days-a-year, in-house therapy services.

Multiple dining venues, including the main restaurants as well as bistros that sever lighter faire and grab-and-go options, such as salads, yogurt parfaits and sushi.

Gracious hospitality service and hotel-like amenities, including a salon and spa, that cater to a Baby Boomer clientele.

All of these features are designed to improve a patient’s outlook on care, increase their satisfaction and ultimately help support their journey toward recovery.

Transitional Care of Arlington Heights will coordinate with are physicians and hospitals to offer critical care pathways that address complex rehabilitation needs with specialty programs that allow for a higher level of care and offer greater satisfaction, improved outcomes, reduced costs and speedy recoveries. To date, Transitional Care of Arlington Heights specialty programs include: Congestive Heart Failure, Orthopedic Injury and Joint Replacement and Advanced Wound Care.
