SD170 Informs Parents of Requirements for Kindergarten Registration

Chicago Heights School District 170 is announcing the following in conjunction with Section 665 of the Illinois State School Code Health Law, which requires a child to have a complete physical examination, including mandated immunizations, upon entering Kindergarten.

Kindergarten Requirements Physical Examination with immunizations are due before the first day of the school year.

Please note that all students registering to enter Kindergarten for the 2024-2025 school year must be five years of age on or before September 1, 2024, and must have a new physical exam, a current immunization record, a new dental exam, and a vision exam.

We strongly recommend you schedule appointments soon with your medical, dental, and eye doctors. The Certificate of Child Health Examination, dental, and vision examination forms must be completed, dated, and signed by the doctor.

At registration in August, parents or guardians must provide:

• Proof of immunizations and exams;
• A certified copy of the child's birth certificate; and
• Proof of residency.

Copies of the required forms and lists of provider clinics may be found on the District 170 website under "Registration" or picked up at the registering school's front office.
