EXPRESS care includes specific tasks such as AM/PM care, bath/shower visits, medication reminders or setup/reconciliation by an RN, safety/wellness checks, transportation for errands or doctor’s appointments, companionship, pre/post hospitalization support (typically used for outpatient surgeries or chemo days) and transitional care back to home.
