Presbyterian Homes Partners With Illinois Bone & Joint Institute in Care Improvement Program
Evanston, IL (March 31, 2014)— Presbyterian Homes’ skilled nursing care and rehabilitation centers have been selected by Illinois Bone & Joint Institute (IBJI) as a partner in its new OrthoSync initiative to improve patient care.
OrthoSync is an IBJI program that seeks to improve patient care by increasing communication and collaboration between all providers involved in the patient recovery process. The OrthoSync program is IBJI’s model for participation in the Centers for Medicare & Medicaid Services (CMS) Bundled Payments for Care Improvement initiative created to improve the quality of health care delivery for Medicare beneficiaries while reducing costs.
Presbyterian Homes’ rehabilitation centers are fully equipped to work with older adults, providing therapeutic support and training for independence in the full range of activities of daily living.

Presbyterian Homes’ rehabilitation centers are among only a few that provide both inpatient and outpatient therapy. Its specially designed rehab centers feature innovative equipment, technology and training protocols that make rehab easier, safe and more tolerable for older adults. Presbyterian Homes' rehabilitation centers consistently receive 4- and 5-star ratings from Medicare.
“We are very excited about IBJI’s participation in the Bundled Payments for Care Improvement initiative,” said David J. Wold, chief operating officer of IBJI. “It’s a critical step towards improving quality while lowering the cost of care we provide.”

Wold continued, “Through IBJI’s OrthoSync, we will work to improve the management of care of our patients in an effort to enhance excellence and efficiencies. IBJI will coordinate patients’ clinical and rehabilitation services across the continuum of care whether the patient is in a hospital, skilled nursing facility, receiving home health services or outpatient rehabilitation services.”
OrthoSync provider partners work together to reduce hospital readmissions, duplicative care, and complications to lower costs. Orthosync also connects health care providers with their patients during their recovery. A web-based application allows IBJI staff and its selected skilled nursing facility, home health agency and outpatient physical therapy partners to be better connected. Providers, patients and key family members or friends can view progress and anticipated milestones.
