Registration Starts for Dist. 124 Elementary School Students

Registration for the 2014-2015 school year at Evergreen Park Dist. 124 elementary schools for new students, those with address changes and children entering kindergarten will start on August 8 by appointment only.

The first day of school for children at Evergreen Park’s four public grammar and junior high schools will be from 8:30 to 11:45 a.m. on August 25.

The before school-extended day program will also start August 25, with the first day of after-school, extended day program beginning August 26.

The early childhood preschool program will start on August 27.

Registration packets can be picked up at the D124 business office (Central Junior High School) at 9400 S. Sawyer Ave.

The resident’s home and cell phone number, as well as the child’s original birth certificate and letter of good standing from the previous school are required upon registration. If the area code of the provided phone number does not match the Evergreen Park 708-area code, the home and/or cell phone bill must be provided showing the Evergreen Park resident’s address.

Proof of residency within Dist. 124 will be required upon registration for new, change of address and kindergarten students. Parents and guardians will be required to provide one of the following approved documents:

Current mortgage papers or statementCurrent homeowners insurance policyCurrent original signed and dates lease listing all tenants and proof of last month’s rent payment, e.g., canceled check or renter’s receipt, landlord’s certificate of inspection from the Village of Evergreen Park and landlord informationAgreement of sale for residential property within the district boundaries with a closing date of 30 days within the date of school registration. An escrow deposit of 10 percent of the actual per diem tuition cost for the 30-school day, time period. The escrow deposit will be refunded upon receipt of closing papers.

In addition to the above, parents and guardians must also produce two of following documents demonstrating proof of residency:

Most recent gas, electric, water, cable television, and/or credit card billVehicle registration Voter’s registration cardCurrent public aid cardRenter’s insurance policyBank statementPayroll stubAutomobile insurance policy

And one of the following showing the resident’s address:

Illinois driver’s licenseIllinois State ID

Required pieces of documentation (e.g., mortgage papers, renters’ receipts, utility bills, etc.) establishing district residency must be dated within the month of registration.

Families that do not own or rent but are living with an Evergreen Park resident will need to complete additional forms, along with the resident they are living with. Please inform the district business office if this applies to you.

Returning students will also be continuing the residency verification process by presenting one of the aforementioned items. Registration packets for all currently registered students were sent home on June 6, containing a fee worksheet and an emergency information/residency verification form.

The fee worksheet and emergency/residency verification forms, along with acceptable proof of residency must be presented at registration.

Payment of school fees or arrangements to pay school fees will take place at registration at the Dist. 124 administrative office 9400 Sawyer Ave. Fees will NOT be accepted by mail.

Hours and dates of registration/fee payments for returning students are as follows:

Monday, Aug. 4 - 9 a.m. to noon; 1:30 to 3:30 p.m.; and 5 to 7 p.m.Tuesday Aug. 5 - 9 a.m. to noon and 1:30 to 3:30 p.m.Wednesday, Aug. 6 - 9 a.m. to noon and 1:30 to 3:30 p.m.Thursday, Aug. 7 - 9 a.m. to noon and 1:30 to 3:30 p.m.Friday, Aug. 8 - 9 a.m. to noon; 1:30 to 3:30 p.m.; and 5 to 7 p.m.

Parents and guardians may pick up their children’s information packets at their child’s respective school starting August 14. Packets will not be turned over without a receipt from the D124 business office and completion of the residency verification form. Please attend one of the dates above prior to August 14 to avoid delays at your child’s school.
