Admissions documents may be downloaded from the District 304 website, be completed at home, and turned in at the time of admissions. Medical, dental, and vision forms can be turned in at admissions. They are required to be submitted before the first day of student attendance.
In addition to the completed admissions paperwork, you will need to bring:

  • Your child’s original birth certificate or a certified copy, which will be copied and returned to you that evening
  • Two documents confirming residency in Geneva, must be property tax bill, recent utility bill showing address, document from closing or rental lease, free of contingencies
  • Parent or guardian’s photo I.D. (ex. driver’s license, passport, foreign government I.D., etc.). This is used for identification purposes only

“We encourage all parents to attend this admissions event as it helps us better plan for space and staffing needs,” according to the District 304 website. “If you are unable to attend the district-wide Kindergarten Admissions Event, please contact the Central Administration Offices at 630-463-3000 after March 1, but as soon as possible, to schedule an appointment to enroll your child.”
