Honoring Your Wishes

“Honoring Your Wishes: Advance Care Planning” will happen from 10 to 11:30 a.m. Thursday, April 16 at Midwest CareCenter, 2050 Claire Court, Glenview.

As part of the 8th Annual National Healthcare Decisions Day, learn about the importance of sharing your wishes with loved ones and how to get started. This program will provide participants with information and education about the various advanced directives and the chance to review the new 2015 Illinois Statutory Power of Attorney for Health Care form.

Speaker Debbie Roberts, an elder law and probate attorney and founder of LifeWrap Solutions, will explain how to have meaningful conversations with loved ones so they are able to best carry out your wishes. You will also get tips on how to consolidate your important records and information so everything is in order in case of an emergency.
