Davis was given injections to calm him down, as he was saying he was not fine and did not know where he was, police said.
A nurse told officers that Davis shoved her as he was trying to get out of his room. She said she had no marks or bruises and did not want to sign a complaint against Davis.
Another employee said Davis hit him in the chest, which caused him to fall, according to the report. The employee said he strained his back and suffered a small bruise on his chest, but that it had gone away, police said. He, too, declined to file a complaint.
According to the report, an emergency room tech was hurt in the right shoulder in the incident. He said Davis yelled, took off his gown and threw punches, police said.
The tech also said Davis ran naked in front of a room that a young girl occupied, according to the report. He said Davis shoved him in the chest. The employee said he grabbed Davis' wrist, while a hospital security guard grabbed the other.
Surveillance video showed Davis walking out of the emergency room and toward a hallway, police said. A number of guards were present, but they did not try to stop Davis, according to the report.
When Davis walked back toward the emergency room, the guards cornered him.
A few days later, the nurse said she wanted to sign a complaint against Davis.
On Feb. 24, a Burr Ridge officer showed up at the Hinsdale police station with Davis.
Davis' bond had been set at $15,000. A relative paid the required 10 percent, or $1,500.
