"I looked down to put it out when I crashed. It happened so fast," he was quoted as saying.
A blacked-out portion of the report apparently contains information about drugs. Following that part, Mikesh was quoted as saying, "It slows down my reaction time, so I don't know."
He denied using other drugs or alcohol, the report said.
Mikesh was driving a 2007 Lincoln that he said he just bought, so it wasn't registered or insured, police said.
The report did not indicate whether any of the occupants of the other cars suffered injuries. But police said they were all outside their cars when officers arrived.
A couple of days after the crash, Mikesh said he was still in a lot of pain, police said.
Mikesh was charged with aggravated DUI, DUI (drugs), driving an uninsured car and failing to reduce speed to avoid a crash.
