LT Residency Verification Forms are Due
The forms have been mailed and must be returned by July 1st.

Every year Lyons Township High School requires every student to verify their residency. This was something new to me after moving back to the area from out of state. LT is a terrific school and provides a wonderful education. The people living in the district pay for this wonderful school in their property taxes. To help protect residents and make sure that only students living in the district are attending LT, the annual residency verification is a must.
The form is quick and easy. A total of three documents must be provided, which can include: current real estate tax bill, a state or federal government issued photo id, utility bill dated within the last 30 days, voter registration card, or receipt for current vehicle sticker. Copies of the documents are required, so please do not send original documents.
The form was mailed to residents in the past week. If you did not receive a form, they are available in the main offices of both campuses: North Campus, 100 S. Brainard Ave., and South Campus, 4900 S. Willow Springs Rd.

In addition to the residency form the mailing included a Student Census Verification form that must be returned by July 1 also. This form asks for various student information that includes emergency contact information, information about any medical conditions, and the OPT OUT for the PTC Directory and Military Recruitment. To opt out of either, simply circle the OPT OUT listed for each on the form.
I’ve already filled mine out and it’s ready to be dropped off or mailed. I’m one of those that will set it to the side and it will get lost in a mound of paperwork if I don’t do it right away. I don’t care for paperwork, but these documents are easy to fill out and took me a total of five minutes to complete.
