What Matters Most? Attend Free Advance Care Planning Sessions At Library

At the end of life, each story is different.

  • What kind of treatment do you want or not want in a medical emergency? 

  • Is your priority to add the most days possible to your life, or to add the most quality of life to your days? 

  • What makes life meaningful to you?

Life Matters Media will provide free, personalized, one-on-one end of life planning assistance every third Thursday of the month this year at Sulzer Regional Library, 4455 N Lincoln Ave., from 10:30-11:30 a.m. Trained facilitators will help you determine “What Matters Most.”

Life Matters Media, a Chicago-based nonprofit, has partnered with Chicago Public Library and 47th Ward Alderman Ameya Pawer to help Chicagoans of all life stages document their end of life care wishes.
