Oak Street Health Opens First Aurora Location

The Aurora location, at 1022 N. Farnsworth Ave., joins Oak Street Health's 26 other primary care centers in Illinois.

AURORA, IL — Oak Street Health opened its first primary care center in Aurora Monday, joining 26 other locations in the state.

The network of primary care centers for adults on Medicare is expanding its access to older adults, and the Aurora facility, located at 1022 N. Farnsworth Ave., will provide a new healthcare option to the 9 percent of residents in the city who are 65 years or older, according to a news release.

"Providing Aurora older adults with a primary care facility is extremely important, especially now in a post-pandemic world," a release said.

At the new center, staff will also be able to pick eligible patients up from their home and bring them to and from appointments, officials at Oak Street Health said. It also accepts various healthcare plans, including traditional Medicare. Behavioral health care, social health support and Medicare education classes are also offered at the center.

"We are excited to expand access to our high-quality, value-based care to members of the Aurora community with the opening of this new center," Ana Cesan, senior regional vice president at Oak Street Health, said in a statement. "The majority of older adults we care for have two or more chronic conditions, making this kind of care so important. As our organization continues its mission to rebuild healthcare as it should be, we look forward to improving patient outcomes in Illinois and helping older adults live their healthiest lives."

Of the 27 centers in Illinois, Oak Street Health, founded in 2012, has several in Chicago, one in Elgin and two in Joliet. Officials said Oak Street Health plans to continue investing in its home city of Chicago and is planning openings in Peoria and Streamwood this year and into 2022.
