Sports Physicals $25 and School Physicals $45

Presence Medical Group at the Healing Arts Pavilion in New Lenox is offering discounted sports and school  physicals now through August 30, 2013.  The cost for Sports physicals is $25 and a school/daycare physical is $45 which includes a physical exam and completion of required forms.  Immunizations, if required, are an additional fee.  Cash,  or credit card accepted at time of appointment - insurance will not be billed.  Call 815-463-0351 option 1 to schedule your appointment .  Weekend and evening appointments available.
