NAIPC Presents: Supporting Seniors to Age in Place - Resources and Community Partnerships

This CEU program, presented in collaboration with the National Aging in Place Council (NAIPC), supports professionals in helping older adults live safely and confidently in the place they call home. Participants will identify the roles of key resources, including senior communities, home health, home services, hospice, and care management. The program will review funding options and evolving reimbursement models, including recent CMS changes, and address strategies to reduce hospital readmissions. Attendees will also develop approaches to educate consumers on end-of-life options, pharmacology, provider scope, and financial considerations. Emphasis will be placed on interdisciplinary collaboration with community professionals including elder law attorneys, home modification specialists, real estate professionals, financial planners, and long-term care advisors to support comprehensive, person-centered aging in place.
