Bremen District 228, Jewel-Osco Partner In Vaccine Drive At OFHS
Oak Forest High School will host a vaccine drive on Oct. 7. A variety of vaccines will be available.
OAK FOREST, IL β In an effort to offer resources to anyone who may need it, Bremen High School District 228, in partnership with Jewel-Osco, is organizing a vaccination clinic on Oct. 7 at Oak Forest High School, located at 15201 S. Central Ave., Oak Forest, IL.
A variety of vaccines will be available for attendees, including the new Bivalent COVID-19 booster. The booster will be available to those who have received the first two doses of a COVID-19 vaccine, or other booster shots, at least two months prior.
Vaccines that will be available include:
β COVID-19 vaccines for 3+ years old
β New COVID-19 Bivalent booster for those 12 years old and up
β Flu regular for 3+ years old
β Flu HD (senior dose) for 65+ years old
β Pneumonia
β Shingles
β Tdap
β Hepatitis A and B
β MMR
β HPV
β Meningitis
β Polio

The wellness clinic will take place from 9 a.m., to 5 p.m. This event is open to the community, and all are welcome to attend. In order to receive a vaccination of any kind, interested participants must register for the wellness clinic through Jewel-Osco, here.

βD228 has hosted vaccination clinics with Jewel-Osco in the past, and is excited to host another this fall,β said Yasmeen Sheikah, D228 Public Relations Specialist. βNot only are all of our students, parents and staff encouraged to attend, but we are also extending the opportunity to our district community members as well.

All attendees must fill out the Informed Consent for Immunization with Inactivated & Live Vaccines form. The form must be filled out the day of the wellness clinic, and a printed copy must be handed to staff prior to receiving a vaccine.
All forms of COVID-19 vaccines are free of charge, regardless if a patient is insured or uninsured. Other vaccinations are fully covered for those with PPO insurance plans (HMO not accepted). PPO, Medicaid, and Medicare B will cover the cost of flu vaccines. If insured, patients must bring their insurance cards.
After receiving the vaccination(s) patients will walk out with at-home COVID-19 tests, free of charge, and 10% off Jewel-Osco coupon(s)βone per vaccination.
Additional information:
β Anyone who is not 18 years of age or older will need a legal guardian to sign their vaccination
consent forms. Parents do not need to be present when the inoculation occurs, but must sign off.
β The wellness clinic will be held on a teachers institute day, no classes will be in session.
β No transportation will be provided.
β Entrances are wheel-chair accessible.
