Oak Forest Barracudas Swim Team Registration 9/7, 9/8 & 9/11

REGISTRATION DATES

• Wednesday, September 7:  6pm–8pm
• Thursday, September 8:      6pm–8pm
• Sunday, September 11:       3pm–6pm *

* New Swimmer Assessment (bring your suit, goggles & towel)

At Oak Forest High School, 15201 S. Central Avenue

Registration Fees: Registration is based on level of swimmer. AAdditional ISI Fees, Volunteer Deposit and Fundraising Commitment apply as outlined below:

Registration Fees:

• Bronze    $325
• Silver      $350
• Gold       $375 

Fundraising Fees:

• $160 total (Charity Mania Football Raffle tickets will be available at registration
• $140 Option buyout (Due at registration)

Volunteer Deposit:

• $75 deposit in separate check to be returned at end of season upon completion of volunteer requirements

ISI/Insurance Fee:

• $60 mandatory insurance fee

Discounted Fees:

• $30.00 off if full amount paid at registration (cash or check only)
• $25 off for each additional swimmer.

Due at Registration:

• At least 25% of tuition
• ISI Fee: $60
• Volunteer Deposit: $75 (in a separate check)
• Fundraiser: Raffle tickets will distributed at registration OR Optional buyout

Note:  If monthly payment is not received on due date, swimmers will be suspended from practice until arrangements are made. No refunds will be given after the first 2 weeks of practice.
