Registration at the Oak Forest High
School Pool, 15201 S. Central Avenue

                     
Sunday, September 7 from 3:30 p.m. to 5:30 p.m.

                  
   Sunday, September 14 from 3:30 p.m. to 5:30 p.m.

 **if you are registering as a
new team swimmer you must be tested.  The test is swimming one length of
the 25 yard pool without having to stop to hold onto the side or line. 
Please bring suit, towel and goggles to registration.  Making the team is
at the discretion of the coaches.  If you are unable to make it to one of
these registration days but are still interested please contact us at
swimofsa@yahoo.com.  Late registration is always accepted.

practice times are as follows.

Bronze 6:30 pm to 7:15 pm Monday
through Thursday

Silver/Gold   7:15 pm to 8:45
pm Monday through Thursday

High School 7:15 pm to 8:45 pm
Monday through Thursday

*Fall/Winter season runs from Mid
September through early February.  **Season starts on 9/15/2014

Registration Fees: Registration is based on level of swimmer. Additional ISI
Fees, Volunteer Deposit and Fundraising Commitment apply as outlined below:

Registration Fees:

Bronze     $325

Silver       $350

High School discounted rate

Fundraising Fees:

·        
$80 total per family

          
Will be hosting a Swim-a-thon in October

Volunteer Deposit:

·        
$75 deposit in separate check (one per family) to be returned at end of season
upon completion of volunteer requirements

ISI/Insurance Fee:

• $60 mandatory insurance fee (non refundable).  ALL
  swimmers must pay this fee.

Discounted Fees(only one may apply):

• $25 off on a second/third swimmer
• $25 off if moving up from our betta lessons program to
  team
• $30 off if paying in full

Due at Registration:

• At least 25% of tuition - Monthly payments are
  available for the remainder of the balance
• ISI Fee: $60
• Volunteer Deposit: $75 (in a separate check-not to be
  cashed until end of season)
• Fundraiser

Note:  No refunds will be
given after the first two weeks of practice.  ISI fees are
non-refundable.  Season starts on Monday, September 15.  Regular practice
schedules will be Monday through Thursday.  If you are not sure if the
swim team is the right fit for you, we offer a two (2) week trial for
$35.  If after the two weeks you decide to stay the rest of what was
required due at registration will be due immediately.
