Name Change of Concord Nursing & Rehab to Aperion Care Oak Lawn and 4 star recognition

Formerly Concord Nursing & Rehab has changed its name to Aperion Care Oak Lawn. This name change comes along with many exciting changes for the facility. CMS (Centers for Medicaid and Medicare Services) has recently recognized the facility as a 4 star facility. Aperion Care is one of the only 4 star facility in the Oak Lawn area!
This recognition comes to facilities that offer quality care to their residents proven with positive clinical outcomes, increased staffing ratios, high number of RN's compared to LPN staff, and good standing with Illinois Department of Public Health surveys. The facility has worked very hard for this recognition.
The facility has also recently become JHACHO accredited which is an additional quality care accreditation.
