Nominations Sought for Oak Park Health Awards

Know any civivally minded health nuts who made a difference in 201o? 

Why not nominate them for a William Fitzsimmons Award, which honor those who've "exceptional contributions to improving the health of Oak Parkers." 

The awards will be doled out by the Oak Park Department of Public Health during Public Health Week, which is April 4 through April 10. 

The deadline for nominations, which can be downloaded here or picked up at , is 5 p.m on Friday, March 25. Completed forms can be mailed to:
