Oak Park and River Forest Join Forces to Launch Health Needs Assessment

Cooperative effort seeks to get pulse on physical, mental health of adults in the area

Through an online survey that spans a wide range of health-related questions, Oak Park and River Forest have launched a community-wide check-up to better understand health needs in the area.

The Oak Park & River Forest Community Health Needs Assessment Survey is open to residents 18 years and older. Responses are requested by April 15. Completion of the survey represents an important investment in the future planning of our two communities’ physical and mental health programs and services, said River Forest Township Supervisor Carla Sloan.

“The assessment is absolutely anonymous and confidential,” said Sloan. “Responses will yield very important information that will help inform our priorities in the future.”

In addition to River Forest Township, the survey is sponsored by the Oak Park Health Department, the Community Mental Health Board of Oak Park Township, Oak Park Township and the Rotary Club of Oak Park-River Forest.

The survey—which takes 10 to 20 minutes to complete, depending on the individual’s answers—is online at https://www.surveymonkey.com/r/OPRFHealthSurvey.

Paper copies of the survey will also be available for those without online access, located at public buildings in Oak Park and River Forest, such as the public libraries, Village Halls, Township Senior Services and Township offices.

The Township efficiently and actively funds and directs the delivery of essential human services in the areas of youth, seniors, mental health and general assistance.
