Orland Township (14807 S. Ravinia Ave, Orland Park, IL.) will be holding an APPOINTMENT ONLY COVID-19 Booster clinic on Friday, Nov. 19.

Orland Township (14807 S. Ravinia Ave, Orland Park, IL, 60462) will be holding a COVID-19 Booster Clinic TOMORROW, Nov. 19!

The Township also asks that those planning on attending any of the clinics wear a short sleeve shirt and arrive in good health.

BOOSTER REQUIREMENTS

  • Must be at least 6 months from 2nd dose in order to receive 3rd dose. (2nd dose date of April 28)
  • Bring completed/signed consent form
  • Bring copy of driver's license
  • Bring copy of insurance card
  • Bring CDC vaccination record card that shows previous 2 shots
