The preliminary investigation determined Izaiah Perez, of Chicago, was driving a Chrysler Pacifica north on Orchard Road, near the intersection with Route 34, when the car veered into the grass. The minivan drove through the intersection and struck a westbound semi-truck, then an eastbound car, police said.

The semi also hit two other westbound cars as a result of the impact, officials said.

Perez and a passenger of the semi were hospitalized with non-life-threatening injuries.

Perez was cited with improper lane usage, failure to reduce speed to avoid an accident and disobeying a traffic control device.

Route 34 was shut down in both directions for about four hours as a result of the crash.
