City Of Palos Heights: Suburban Cook County In-Home COVID-19 Vaccination Contact Form

The Cook County Department of Health (CCDPH) offers in-home COVID-19 vaccination services to reach individuals who find it difficult to go to COVID-19 vaccination sites to get the vaccine (1st, 2nd, and 3rd doses and boosters). Please note that CCDPH vaccination sites are ADA accessible, and this program is specifically for suburban Cook County residents who find it difficult to leave their home to receive a vaccination. Individuals living outside suburban Cook County are not eligible for this program. Individuals in Chicago, Evanston, Oak Park, Skokie, and Stickney are currently not eligible for this program as they may provide these services through their own certified health departments.
