Tri City Health Partnership Receives $35,000 Grant from the CVS Health Foundation
Grant Awarded to Improve Access and Affordability in St. Charles
St. Charles' Tri City Health Partnership recently received a $35,000 grant from The CVS Health Foundation in partnership with the National Association of Free & Charitable Clinics. The grant was awarded as part of the CVS Health Foundation’s multi-year, $5 million commitment to address the need for more accessible, coordinated health care in local communities around the country like St. Charles.
The grant Tri City Health Partnership received from the CVS Health Foundation will enable them to fund their Coordinated Chronic Care Initiative to assist clinic patients in the management of their chronic diseases. The initiative will provide increased access to medications and supplies, comprehensive health education, medical treatment and monitoring and documentation of the treatment of chronic diseases such as asthma, diabetes and hypertension.
“We are committed to helping people on their path to better health and building healthier communities,” said Eileen Howard Boone, president of the CVS Health Foundation. “Through our ongoing partnership with the National Association of Free & Charitable Clinics, we’ve been able to address the critical need for increasing access to care, improving care coordination and managing chronic conditions.”
