COVID-19 Dose 2 Clinic Saturday
Dose 2 will be available to children (aged 5-11) who received Dose 1 of the Pfizer COVID-19 vaccine at Central Middle School on November 13
If your child (aged 5-11) received Dose 1 of the Pfizer COVID-19 vaccine at Central Middle School on November 13, this is a reminder that Dose 2 for your child will be available this Saturday, December 4.
No additional appointment is needed to receive Dose 2. Simply return to Central Middle School (18146 S. Oak Park Ave, Tinley Park) at your original appointment time. A second Jewel/Osco consent form must be completed; copies will be available at the clinic.
Please bring your child's CDC Vaccination Card with you.
