Cook County Collecting Storm Damage Questionnaires
Cook County Department of Homeland Security collecting information from residents and businesses related to flood damage on Aug. 22.

The Cook County Department of Homeland Security and Emergency Management is collecting information from homeowners, renters and businesses related to flood damage and costs from local jurisdictions in order to provide it to the Illinois Emergency Management Agency. The information will be used to preliminarily determine whether the area is eligible for federal disaster assistance. Updates regarding this process will be provided to residents as they are received.
If your home or business sustained damage in the storm event on Friday, August 22, please fill out an Initial Damage Assessment Form and e-mail it to Emergency Management Director Patrick Kenny at pkenny@wsprings.com. You may also fax the document to (708) 246-4871, drop the form off at Village Hall, 740 Hillgrove Avenue or fill out the online form.
Completed forms are due by Friday, September 5.
