The Cook County Department of Public Health is now inviting people to register online to receive information about when and where they may be able to receive COVID-19 vaccines.

The Cook County Department of Public Health (CCDPH) is using the information collected from this survey to gather information from individuals who would like to receive regular updates about when and where they can receive the COVID-19 vaccination. Please note that the vaccination supply is still limited. High risk individuals as defined by the CDC are currently being prioritized.
