The School City of Hammond, in partnership with the Indiana Department of Health and FEMA, is providing families with transportation to the mass COVID-19 vaccination clinic, located at the former Roosevelt High School in Gary.

Current students age 16 and older are eligible for both the vaccine and transportation to and from their home school. Transportation dates are Friday, April 23, and Friday, April 30.
To register for the vaccine and transportation, parents or guardians can call their student's home school nurse or athletic director. Students over the age of 18 are eligible to register themselves at their school.

A completed consent form must be brought to the vaccination site on the day of vaccination. Insurance information is not required and should be left blank on the completed form.
Facemasks are required on the school bus and at the vaccination site. Social distancing guidelines will be followed on the school bus.

La ciudad escolar de Hammond, en asociación con el Departamento de Salud de Indiana y FEMA, está proporcionando a las familias transporte a la clínica de vacunación masiva COVID-19, ubicada en la antigua escuela secundaria Roosevelt en Gary.
