Coroner Reports Man’s Head, Torso Were Crushed during Fatal Amazon Center Accident

PLAINFIELD, IN — The Hendricks County coroner reports an Indianapolis man was the victim in fatal forklift accident at the Plainfield Amazon facility Sunday afternoon, according to Fox59. The news outlet reports information released from the coroner said 59-year-old Phillip L. Terry of Indianapolis sustained multiple injuries from the accident at the 800 Perry Road facility, citing injuries to his torso and head as the most significant. According to Fox59, Terry's cause and manner of death will be available in four to five weeks, and the state’s Occupational Safety and Health Administration (OSHA) will also conduct an investigation.
