FLU VACCINE CLINICS

Dates: Wednesday, September 29 and October 20
Time: 4:00 p.m. – 8:00 p.m.
Location: Urbandale High School Commons (please use the main entrance)
Eligibility: UCSD Students, Staff, Parents, Siblings, and Community
Registration: Flu Vaccine Registration (Note: It is highly recommended that you visit the website and pre-register to make your visit faster!)
Cost: Most major insurance plans accepted. Please bring your insurance card. If you do not have insurance, the cost is $25 or what you can pay.
Consent: Parent/guardian of students under 18 must be present and sign consent
