New Opportunity for Health Insurance
Need Health Insurance? This is your chance to receive what suits your life best

Are you in a bind for health insurance? According to healthcare.gov, there’s an opportunity for you to receive the plan that works best for you this Saturday.
When – Sat. December 13 from 9:00am to 1:00pm

The Polk County Health Department, Planned Parenthood of the Heartland, Visiting Nurse Services of Iowa and Mercy Medical Center will graciously host a fair to assist in signing you up for the best health care plan for you.
Rick Kozin, director of Polk County Health Department, said that “there are still many Polk County residents without health insurance.” He also noted that this fair will be “an opportunity for them to discuss their options with trained professionals who understand their family’s health insurance concerns.”

If you would like your coverage to start by January 1, 2015, then you must enroll in a plan by December 15, 2014. If you enroll after December 15, 2014, then coverage will only start the next month or the second month, depending on your enrollment date. Be sure to get your plan soon because open enrollment ends February 15, 2015.
If you enrolled last year through Marketplace, Kozin states that you need to start reviewing current plans to decide if you want to renew your current plan or enroll in a different one.
Here’s what you need to bring in order to complete the enrollment application at the fair:
- Current ID
- Social Security Card
- 2013 Income Tax Returns
- If working, last pay stub
- Every client must have an email address to receive notice of coverage decisions and requests for additional information
