"For our COVID Task Force and Response Team to effectively develop and deploy mitigation measures, we need additional data about the vaccination status of our campus community. We ask all members of the WKU community to complete the Vaccination Status Reporting Form located at https://app.wku.edu/vaccine/.
The COVID-19 Task Force will use aggregate level information to continue its work to control viral spread on campus. Individual vaccination status information gathered through the form will not be shared with an individual’s instructor(s), advisor(s), or supervisor(s).

Additionally, we are pleased to announce that WKU has created a COVID vaccination contest which offers incentives for members of the WKU Family who have been vaccinated against COVID-19. Drawings will begin the week of August 30 and will continue for 5 weeks. Individuals who have been vaccinated against COVID-19 at any time are eligible to win prizes.
Prizes available for vaccinated students:
- Undergraduate, full-time, in-state tuition scholarships ($5,496 each; 5 available)
- $500 book scholarships (5 available)
- $100 WKU Store Gift Cards (100 available)
