The City of Portland’s Public Health Division is pleased to announce that it is seeking input for its Community Health Improvement Plan. Portland Public Health invites and encourages all those who live, work, learn, and play in Portland to complete the 2021 Community Health Improvement Survey. The survey is a health opinion survey that gathers information about the health and well-being of Portland and helps identify ways to address those needs and concerns.

The survey is available online and via telephone through August 31, 2021 and has been translated into French, Portuguese, Somali, Spanish, and Vietnamese. To complete the survey via telephone, interested participants can call the Portland Public Health Division at 1-207-874-8919.

Residents and interested parties are also encouraged to check out the Community Health Improvement Planning project page to read more about the process and sign up for project alerts.
