FamilyID is a secure registration platform that provides an easy, user friendly way to register for athletics. Once your profile has been set up, it can be used to sign up multiple children for multiple teams.

How to Get Started with FamilyID for Athletic Team Registration

Pre-Participation Physical Form Is Available on FamilyID

Physical form must be completed after June 1 for athletic programs in the current school year and must be completed by a certified physician. Parents scan and upload the completed documents into Family ID (or use a “smart” phone to take a picture for upload).
