Living Well is a chronic disease self-management workshop (CDSMP) from Stanford University.  Workshops meet for two and one-half hours, once a week for 7 weeks.  Participants will make weekly action plans, share experiences and help each other solve problems they encounter in creating and implementing their self-management program.
