To register for a table please complete the registration form attached to the flyer and turn in by 9:30 on Saturday 5/11/13.

Instructions

1. Fill out the information below  and return by May 10, 2013. Late registration is available until 9:30am May 11, 2013.

2. In order to reserve your table, the table fee of $25.00 needs to be paid by 9:00am May 11, 2013. All Sales are finale*.

3. On May 11, 2013 bring this completed form (or a copy) to Mrs. Que. The table fee will be collected at time listed above.

Note: If you would like to make a donation please stop by our donation table at the event site.
