Medicaid for hundreds of thousands of dollars in mental health telehealth services that were never provided or were inflated. Investigators say the scheme involved systematically overbilling call times and even fabricating sessions, including with an undercover FBI agent, leading to more than $250,000 in losses before the case was brought.
