FALL RIVER, MA — A 16-year-old from Fall River is the latest addition to the state police "most wanted" list.  Jayden Smith, 16, of Fall River, shot and killed Jerrod Cohen, 29, in Fall River on July 11, police said.    "Smith is reported to be a member of the Gangster Disciples street gang and should be considered armed and dangerous," state police said in a statement.     Smith is 5 feet, 8 inches and 160 pounds. He has black hair and brown eyes. He goes by "Smitty" and has a tattoo of a six-pointed star on his hand.    A $1,000 reward is being offered by the Bristol County District Attorney's Office and Fall River police for information that leads to Smith's capture.    Anyone with information on Smith's whereabouts is asked to contact state police at 1-800-527-8873.    The other suspect in Cohen's murder, Joel Lopez, 16, of Fall River, was arrested in July.  <figure class="r-embed"><blockquote class="twitter-tweet" data-lang="en">The MSP Violent Fugitive Apprehension Section added JAYDEN W.
