Residential Program Nurse (RN) - Brain Injury Services

Overview

The Residential Program Nurse provides direct nursing oversight and management of medical and care components through training and supervision of both residents and staff in all aspects of medical and medication management. This role collaborates with the Residential Program Director in the implementation and follow-through of the individualized service plans and documentation in accordance with the mission and regulations of the program.

Responsibilities

• Take a lead role in assessing potential program participants at his or her residential site; identify the program participant’s needs and functional abilities.
• Participate in intakes and periodic team assessment with the applicant, family member/guardian and Residential Program Director to determine whether the participant can manage their personal care services.
• Assist residents with all aspects of medical management including physicals, referrals, prescriptions, appointments, and treatments.
• Coordinate with residents, family members, physicians, other medical providers, and site team regarding all medical care and changes in status, treatment, or medication.
• Implement, oversee, and supervise medication management in accordance with agency policy in a manner consistent with funder regulations.
• Ensure Physician’s Authorization for each GAFC resident twice annually per Medicaid site policies (if applicable).
• Prior to an applicant’s admission to the program, collaboratively complete a Medication Assessment and develop an Emergency sheet, Medication List, individualized ADL sheet and a risk plan.
• Develop an individualized Emergency Contact sheet, which includes the participants’ identifying and medical information.

Qualifications

• Current license to practice in Massachusetts as a Registered Nurse and current certification in CPR/Basic Cardiac Life Support required. Graduate of an Accredited School of Nursing with at least 3 years’ clinical experience required. Experience with individuals with brain injuries in a community setting preferred.
• Ability to correlate clinical data with resident's medical and nursing care plan.
• Ability to communicate effectively both verbally and in writing and ability to use good judgment.
• Ability to set priorities and handle multiple simultaneous tasks, organizing, prioritizing, planning, sequencing and completing tasks independently.
• Basic computer knowledge.
• Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients.
• Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
