emailing at parentt@grafton-ma.gov
Leave your name, telephone number and time preference.
Clinic open for ages 6 months and up / Grafton residents ONLY
Please wear short sleeves and bring ALL insurance cards.
Please visit www.grafton-ma.gov under Health Department
   to print and fill out your insurance information and bring
   it with you to the clinic.
