Commonwealth Care Alliance's Senior Care Options Program Expands in Hampden, Hampshire, and Plymouth Counties
Commonwealth Care Alliance's Senior Care Options program will be expanding to Hampden and Hampshire Counties, parts of Plymouth County
BOSTON —February 12, 2015 — Commonwealth Care Alliance’s Senior Care Options program will be expanding to include full county coverage in Hampden and Hampshire Counties and select zip codes in Plymouth County, effective January 1, 2015.
Commonwealth Care Alliance’s Senior Care Options (HMO SNP) program provides and coordinates all services covered under Medicare and MassHealth for individuals 65 and older, in addition to other benefits deemed necessary by each member’s primary care team. With Senior Care Options, each member is assigned a dedicated team of healthcare practitioners chosen to meet his or her individual needs. A team may include medical specialists, physician assistants, nurses, nurse practitioners, behavioral health practitioners, social workers, and other professionals, all working collaboratively to provide ongoing health management, early intervention, and urgent care.
CCA’s Senior Care Options plan has been consistently rated among the highest quality Medicare Advantage plans nationwide. U.S. News & World Report rated the plan among the “Best” Medicare Advantage plans for 2015, and the Centers for Medicare & Medicaid Services (CMS) awarded the program a 4.5 star quality rating out of a possible 5 stars for the fourth consecutive year.

Care Alliance is a “consumer-governed” organization offering a full spectrum of medical and social services for people with complex needs covered under Medicaid and for those “dually eligible” for both Medicaid and Medicare. Commonwealth Care Alliance offers the Senior Care Options Program (HMO SNP) for individuals ages 65 and older, who have Medicare and MassHealth Standard or just MassHealth Standard alone, and One Care: MassHealth plus Medicare, for dual eligible individuals ages 21 to 64. These comprehensive health plans provide all the services covered under Medicare and MassHealth, and other benefits as determined necessary by an interdisciplinary primary care team. Services are coordinated by the primary care team and are accessed through Commonwealth Care Alliance’s preferred provider network. Senior Care Options Program is a health plan with a Medicare contract and a contract with the Commonwealth of Massachusetts/EOHHS. Enrollment is voluntary.
