The Seekonk Public Schools will once again be partnering with Brewster Ambulance to provide a free COVID-19 vaccination clinic. This next clinic will be solely for students ages 5 to 11. The vaccination clinic will be held at the Hurley Middle School gymnasium and will take place on Thursday, December 2nd with the second dosage being administered on Thursday, December 23rd. Students participating in the vaccination clinic must have a parent or legal guardian with them at the clinic, and the parent or legal guardian must sign the two attached consent forms from Brewster Ambulance and the Commonwealth of Massachusetts. Students participating in the vaccination clinic must bring both consent forms to the clinic on December 2, 2021.Parents/guardians who would like to have their child vaccinated through this program are asked to pre-register by completing an online form so that Brewster Ambulance can gauge vaccine supply amounts and scheduling.
