WALTHAM, MA — The Waltham Health Department will be hosting a Moderna booster shot clinic from Monday, Jan. 3 through Friday, Jan. 7 at Government Center.
The clinic will be open daily from 9:15 to 11:30 a.m. for individuals age 18 and older.
To sign up for a time slot, click here. Note that appointments are not confirmed until you have submitted a pre-screening form by email or spoken with a nurse to complete pre-screening.
