Year-Long Diabetes Prevention Program Starts March 6!

Rochester organization wants you to get healthy!

Practice Transformation Institute (PT) in Rochester, a non-profit organization providing continuing medical education and customized learning programs that improve health outcomes, wants to improve your health and the health of family members and friends in 2018!

The National Diabetes Prevention Program (DPP) teaches healthy lifestyle habits in a small group setting to help participants prevent diabetes. We use the CDC national curriculum which includes sessions on eating healthier, increasing physical activity, managing stress and changing behaviors. The year long program meets for one hour a week for 16 weeks, twice a month for 2 months, then once a month until the year is completed. The program’s goals are healthy lifestyle changes, 5% weight loss and 150 min/week of physical activity (can be walking!) which helps prevent diabetes.

Some of the criteria to attend the program are 18 years or older and BMI greater than 24. If you've been told you have abnormal labs such as an A1c of 5.7-6.4 or fasting glucose 100-125, we want you. A history of gestational diabetes or increased risk for diabetes due to family history is also another item which would make you qualify.

Class will be held in the Country Creek Medical Building (Silver Bell and Adams), 4986 N. Adams Roads, Rochester in our large conference room starting Tuesday, March 6 from 1 - 2 PM. We will be having an information session on Tuesday, February 6 at 1 PM and Tuesday, February 27 at 1 PM to explain the program and answer questions.
