OCHN release Request for Quotation for TV Enclosure
OCHN is requesting proposals for two tv enclosures with warranty and installation for both (RFQ 719-2022).

Oakland Community Health Network is requesting proposals from qualified proposers for the following items:
TV Enclosure (Quantity of 2)
· For 60” screens. Water-proof, Dust-proof, & Tamper-proof -- NEMA 4 / 4X. Strong, Welded Steel Construction w/ Hinged Door, Built to Last. Shatter-proof Clear or Anti-Glare Polycarbonate Window. German Engineered, Keyed Compression Locks. 110 CFM Axial Fan and Filter System. Filter is Easily Cleaned or Inexpensively Replaced. Internal, Adjustable VESA Mount Bracket (fits all TV’s). Mounts Directly to Wall w/o need for Clunky, Exterior Bracket

CERTIFICATION OF NON-COLLUSION: Vendor warrants under penalty of perjury, that its bid was arrived at independently and without collusion aimed at restricting competition. GOVERNING LAW: The resultant contract shall be construed in accordance with the laws of the State of Michigan.
Requirements for Quotation: Quotations must be submitted on attached form only to: winstonj@oaklandchn.org. Unit prices must be fixed and firm and must include shipping and delivery. Items are subject to review by OCHN. Vendor shall state list price, unit price, and total on each description. Please provide lead time.
OCHN participates in the following cooperative purchasing organizations: MIDeal, OMNIA Partners/U.S. Communities and NASPO.
To be eligible to submit a proposal, a completed W9 Form and Request for Quotation must be emailed to Juanita Winston, at winstonj@oaklandchn.org, for receipt by 4:00 p.m. EST, June 16th, 2022.
