Abbott BinaxNOW COVID-19 Antigen Self-TestKit – 2 tests per kit or equivalent. If
equivalent, vendor shall state make, model and manufacturer and provide spec sheet.
*More kits may be purchased based on cost.
Please provide price break information for up to 5000 kits so a determination can be made
regarding the number of kits.

Qty of 1150 KITS

All price quotes must be firm, i.e., no price adjustments are allowed. All quotes are deemed F.O.B. DESTINATION. QUOTED PRICES SHALL INCLUDE ALL COSTS INCIDENTAL TO PROVIDING THE GOODS SPECIFIED, INCLUDING FREIGHT, PACKAGING, MATERIALS, LABOR, FEES, OVERHEAD, AND PROFIT.

Payment terms are Net 30 Days unless a discount is offered for prompt payment.
NOTE: Oakland Community Health Network (OCHN) is sales tax exempt. Michigan sales tax exemption form will be submitted at time of order.

Requirements for Quotation: Quotations must be submitted on attached form only to: lewisj@oaklandchn.org. Unit prices must be fixed and firm and must include shipping and delivery. Items are subject to review by OCHN. Vendor shall state list price, unit price,
and total on each description. Please provide lead time.

Quote must be completed by 3/24/22 no later than 4:00 p.m.
