Provided by physicians from the Beaumont Family Medicine Residency Program and Center. Each sports physical includes height and weight check, blood pressure, vision screening and a general physical exam (cardiovascular and musculoskeletal exam).
Each student must bring a completed form, signed by a parent or guardian, which is obtained from the school/camp that requires the physical.
