OCHN Request for Proposal for services to children with SED
OCHN is seeking proposals in the local community who wish to deliver services and supports to children with Serious Emotional Disturbances.

Oakland Community Health Network (OCHN) is seeking proposals from interested parties in the local community who wish to deliver services and supports in accordance with the service model to children with Serious Emotional Disturbances (SED), ages pre-natal to 6 years, 6-21 years, and their families.
OCHN utilizes a network of contracted Core Provider Agencies (CPA) to provide services to eligible persons residing in Oakland County. The successful proposer must have the ability to provide SED services as outlined in the Michigan Medicaid Provider Manual (MPM), including but not limited to: Outpatient Therapies, Case Management, Home-Based Services, and Wrap-Around.
To be eligible to submit a proposal, a completed Confirmation of Interest Statement must be emailed to Caroline Babb-Leahey, babb-leaheyc@oaklandchn.org, for receipt by 5:00 PM EST, August 17, 2022.
