Opportunity Partners, a Twin Cities nonprofit disability organization, has recently expanded its hoMEbase apartment program to Coon Rapids (hoMEbase-Woodland) after successfully launching a location in Hopkins (hoMEbase-Westside) in 2011. The hoMEbase program consists of several units of an existing, centrally located apartment complex which are then rented out to people who enroll in Opportunity Partners' services. One of the units becomes an office for on-site staff members who provide independent living training and assistance as needed.
Either single or shared apartments are available; Opportunity Partners provides daily check-ins, organization of social activities, group meals, transportation and assistance with meal planning or banking when needed. For added safety and security, advanced technology such as door monitors, web cameras and similar safety nets are provided. The program allows residents the freedom to live independently but with the security of professional staff nearby.
The GHR Foundation provided three years of funding to help Opportunity Partners innovate, implement and develop hoMEbase. GHR provides funds and works with social service agencies to afford them time and opportunity to create and innovate, to leverage resources for even greater impact, and to touch the lives of those they serve in more powerful ways. The vision of the foundation is to assist those who provide sustainable solutions to the world's most pressing issues.

Individuals are charged the state-legislated room and board rate, which includes rent, utilities, and grocery, phone and laundry allowances. Apartment openings are currently available at both hoMEbase locations. For more information, contact the hoMEbase program managers:
